Job Summary
We are seeking a dedicated and compassionate Registered Nurse Case Manager to join our healthcare team. The ideal candidate will have experience in various clinical settings, including hospice and home health services . This role is essential in coordinating patient care, ensuring effective discharge planning, and providing support for patients with complex medical needs. The Registered Nurse Case Manager will work closely with healthcare providers to deliver high-quality care and improve patient outcomes. This position will require some on call for the area.
Duties
- Assess patient needs and develop individualized care plans based on clinical evaluations.
- Coordinate outpatient services and facilitate transitions of care for patients discharged from the hospital.
- Monitor vital signs and manage ongoing health assessments to ensure optimal patient health.
- Collaborate with interdisciplinary teams to provide comprehensive infectious disease care and medical-surgical support.
- Conduct regular follow-ups with patients to evaluate their progress and adjust care plans as necessary.
- Educate patients and their families about health management, treatment options, and available resources.
- Document all patient interactions, assessments, and changes in condition accurately in medical records.
- Participate in quality improvement initiatives to enhance service delivery within for hospice services
Experience
- Current Registered Nurse (RN) license in the state of practice.
- Previous experience in acute care settings or as a case manager preferred.
- Familiarity with discharge planning processes and hospital medicine practices.
- Strong clinical skills in managing patients with complex health conditions.
- Excellent communication skills for effective collaboration with healthcare teams and patient education.
- Ability to work independently while managing multiple priorities in a fast-paced environment.
