JOB TITLE: LVN Lead Charge Nurse
We are a 6 bed residential care facility, specializing in care for developmentally disabled individuals with nursing needs.

This individual will support and practice our mission and core values, exercise the nursing process, be innovative and practice effective communication and collaboration in our system.
Duties:
- Providing for the safety and well-being of clients both physically and emotionally.
- Ability to perform assessments and document appropriately.
- Carry out client's Individual Program Plan by performing routine nursing duties.
- Assist in scheduling medical appointments and maintain a current medical appointment calendar.
- Oversee caregivers and medication techniques.
- Review daily notes.
- Record new/changed physician orders.
- Responsible for the admissions of new residents to include all assessments, reassessments, and documentation.
- Notifies physicians and/or family members of any change in resident’s health and provides proper documentation.
- Document daily task related to resident care and functional abilities.
- Administer medication and treatments as prescribed.
- Work under the supervision of the RN and Administrator.
- Attends all required training, in-service, and staff meetings.
- Strives to maintain a safe working environment through the prevention of accidents, the preservation of equipment, and the achievement of safe working practices.
- Performs other duties as assigned.
Shift times weekdays 1-9 pm, weekends 12-8 pm, rotating weekends.
Requirements:
- Valid California LVN License
- Exp. with g-tubes, catheters, ostomies
- Physical within the last 6 months
- Negative TB test
- BLS
- Clear criminal background
- Patience and flexibility!
- Works well with a team with high-quality people skills and good leadership!
- Ability to supervise care staff.
- Ability to communicate with physicians, pharmacies, families, and community staff.
- Reliable and Punctual!
- New Grads acceptable.
Job Type: Full-time
Pay: From $34.00 per hour
Expected hours: 40 per week
