Job Description
Patient Assessment and Eligibility
Assume primary responsibility for determining patient eligibility for care based on State and Federal guidelines.
Care Coordination
Obtain data on physical, psychological, social, and spiritual factors that may impact patient and family needs, and initiate intervention by other members of the interdisciplinary team.
Nursing Care
Effectively utilize resources to assist in meeting patient needs, provide regular visits to reassess patient eligibility, needs, and update plan of care.
