Responsibilities
The Hospice RN will be responsible for:
- Assuming primary responsibility for patient and family caseload, including assessment, determining eligibility, admission processing, planning, implementation, and evaluation phases of the nursing process.
- Obtaining data and assessing necessary information from patient physical, psychological, social, and spiritual factors that may impact the patient and family's needs and initiating intervention by other members of the interdisciplinary team.
- Initiating communication with the attending physician, interdisciplinary team, and other agencies for the purpose of coordinating optimal care.
- Providing regular visits to re-assess the patient's needs and update the plan of care.
Requirements
To be successful in this role, you must have:
- A graduate degree from an accredited registered nursing program.
- A valid license as a registered nurse in the state of practice.
- Current CPR and First Aid Certification.
- Knowledge of quality assessment and assurance procedures.
- A valid driver's license and proof of insurance.
