What are the responsibilities and job description for the RN Case Manager position at Serenity Hospice and Home?
Serenity Hospice and Home seeks a compassionate and dedicated RN Case Manager passionate about providing exceptional care! This position would work Monday through Friday, with one weekend of on-call a month. Our RN Case Managers provide care in patient’s homes and must possess the capability to drive. Mileage is reimbursed.
Key Responsibilities:
- Conduct comprehensive assessments of patients referred to hospice care, ensuring a seamless and supportive admission process.
- Collaborate with interdisciplinary teams to develop personalized care plans that honor the unique needs and wishes of each patient.
- Provide education and emotional support to patients and their families, helping them navigate the transition to hospice care with dignity and understanding.
- Maintain accurate and timely documentation in accordance with regulatory and organizational standards.
Qualifications:
- Current RN license in good standing.
- Experience in hospice or palliative care preferred, but not required.
- Strong clinical assessment and critical thinking skills.
- Excellent communication and interpersonal abilities.
- A compassionate and empathetic approach to patient care.
- Detail oriented individual who is highly organized.
