Job Description:
Serenity Hospice is seeking a compassionate and skilled Certified Registered Nurse Practitioner (CRNP) to join our exceptional team. As a CRNP, you will play a critical role in ensuring the highest quality of end-of-life care for our patients and their families. Your primary responsibility will be to conduct face-to-face visits with hospice patients to evaluate their eligibility status in accordance with CMS Hospice Eligibility Guidelines.
Key Responsibilities:
- Conduct face-to-face visits with hospice patients to assess eligibility based on CMS guidelines.
- Collaborate and communicate with the Hospice Medical Director and/or the patient’s attending physician to develop and adjust patient care plans.
- Provide clinical expertise to the interdisciplinary hospice team to support optimal patient care.
- Authorize medication orders as applicable per state laws and patient needs.
- Ensure documentation is accurate and timely to meet regulatory and organizational standards.
- Offer guidance and recommendations to the hospice care team on symptom management and patient care strategies.
- Participate in team discussions, IDG meetings, and care planning sessions as needed.
- Work primarily from an office setting, with occasional travel to patient homes or care facilities within a reasonable distance.
Qualifications:
- Current and valid CRNP license in Pensylvania.
- Experience in hospice or palliative required.
- Strong understanding of CMS Hospice Eligibility Guidelines.
- Ability to work effectively in a team-oriented, interdisciplinary environment.
- Excellent communication, documentation, and organizational skills.
- Willingness to travel as necessary for patient visits (within reason).

Expected hours: 25 – 40 per week
LOCATION: Serenity Hospice Operates four (4) Care Centers in PA. Must be able to work out of one of those Offices.
