Job Title: Full-Time Hospice RN Case Manager 

• $3000 sign-on bonus

Location: Wyoming Valley, PA 

Servicing - Lackawanna & Luzerne PA Counties

Position Overview: As a Full-Time Hospice RN Case Manager at Serenity Hospice PA, you will play a vital role in delivering comprehensive and compassionate care to our patients. Working in our supportive and staff-oriented environment, you will have manageable caseloads, allowing you to provide personalized attention and make a meaningful impact on the lives of patients and their families during their end-of-life journey.

Responsibilities:

• Collaborate with the interdisciplinary team to develop and implement individualized care plans for hospice patients

• Conduct comprehensive assessments of patients' physical, emotional, and spiritual needs

• Coordinate and manage patient care, ensuring the highest level of comfort and quality of life

• Regularly assess and reassess patients' conditions, adjusting care plans as necessary

• Administer prescribed medications and treatments in accordance with physician orders

• Educate patients and their families about disease progression, symptom management, and end-of-life care options

• Provide emotional support and counseling to patients and families, fostering a compassionate and understanding environment

• Maintain accurate and timely documentation of patient care activities

• Adhere to all applicable laws, regulations, and company policies to ensure compliance

Qualifications:

• Valid Registered Nurse (RN) license in the state of Pennsylvania

• Minimum of one year of hospice experience

• Proficiency in performing comprehensive physical assessments and managing pain and symptoms

• Strong interpersonal and communication skills

• Ability to work collaboratively as part of a team and autonomously

• Compassionate and empathetic approach to patient care

• Excellent organizational and time management abilities

• Preferred proficiency in electronic medical record (EMR) systems
