Serenity Hospice Care is looking for an enthusiastic and organized Bereavement Social Worker for our Northern Territory hospitals. This position is full time, Monday through Friday from 8 : 30am to 4 : 30pm.
Responsibilities :
- Assist with performing the social services section of the assessment process as well as provide on-going care to patients and their families as needed.
- Assist with performing bereavement services (calls, mailings, documentation).
- Provide updates to the bereavement plan of care as necessary.
- Attend Interdisciplinary Team meetings as needed.
- Managing and completing all psych-social assessments
- Educating the patient and family about services
- Performs other duties as assigned.
Qualifications :
This position is full time, Monday through Friday from 8 : 30am to 4 : 30pm.
