Social Worker

Job Summary

This contract position is located at Serenity Manor Adult Day Services in Edgewood, MD. This position is a contract position and applicant must be able to bill clients directly via Medicare/Medicaid/Private Insurance. The Social Worker is responsible for assessing the psychosocial, equipment, educational, and/or financial needs of the Day Care clients/participants. Candidate must be available to assess and intervene in situations including but not limited to: abuse, vulnerable adult, traumas, bereavement, chronic illness, new diagnosis, catastrophic diagnosis, financial matters, bioethical issues and guardianship/commitment. Position is responsible for providing social work services to persons of all ages, their families, and other people significant to the persons served in the assigned unit/department. Provides for/assists with timely discharge planning.

Compensation

Must be able to bill clients privately via Medicaid/Medicare/Private Insurance. Please disregard pay as compensation comes from direct billing.

Schedule

Part-time

One to two days a week if needed. Position is open to set their own schedule within reason and based on the needs of the clients.

Duties and Responsibilities

• Coordinate and implement individual, group, and family counseling in conjunction with plan of care goals.
• Inform participants and their families or caregivers of available community services and refer participants, as necessary, to agencies providing these services.
• Provides crisis intervention and emotional support to participants and families on psychosocial issues.
• Write monthly progress notes in participant’s record.
• Be responsible for documenting the follow up of all absences.
• Complete an admission social work assessment and mini mental status exam within 10 days of participant’s admission.
• Complete monthly ADCAP assessment.
• Coordinate Psychiatric F/U and schedule initial therapist evaluation and Psychiatrist evaluation.
• Complete POC for every problem participant exhibits and evaluates them every 90 days.
• Coordinate discharge planning and refer participant to outside agencies.
• Reports all suspicions of abuse/neglect to Adult Protective Services and documents accordingly.
• Communicate with Care Provider all concerns or changes in POC.
• Coordinate Care conferences with Care Providers and inter-departmental care team.
• Coordinates referrals to outside community resources as needed.
• Assists Center Director with marketing Serenity Manor Adult Day Services.
• Coordinates admission process and receives all needed information form referring agency.
• Documents all participant housing changes in Center record and follows up accordingly.
• Performs basic administrative tasks related to the job; updating community resource lists, statistics, reports, and other routines as required.
• All other duties assigned by Center Director.

• Qualifications
• Licensed medical/clinical Social Worker in compliance with licensing in the state of Maryland.
• Minimum three (5) years’ experience in a professional capacity providing services to adults in a health care setting, adult day center, assisted living/nursing home, or related field.
• Strong verbal and written communication and facilitation skills.
• Strong assessment, time management, and critical thinking skills.
• Advanced computer skills.
• Skills and knowledge of human behavior and clinical aspects of medical Social Work.
• Ability to interact effectively with a variety of people and situations at all levels of the organization.

Pay: From $194.14 per day
