Coordinates medically related social services to nursing home residents overseeing the admissions and discharge process.
Duties include :
- Execute a dmitting individuals to facility working with local referral sources and community members
- Perform cognitive and psychosocial assessments and care planning
- Discharge planning to include making referrals to community resources
- Providing emotional support and education to residents and their family members
- Advocating for residents’ rights
