Duties and Responsibilities :

  • This position coordinates medically related social services to nursing home residents.
  • Care planning

  • Advocating for residents’ rights
  • Providing emotional support to residents and their family members.

Qualifications :

  • Must possess a bachelor’s degree in a related human service field.
  • Experience working in a nursing home setting preferred.
