Wesley Gardens Nursing Home is looking for a PER DIEM Physical Therapist Assistant (PTA) to join our seasoned professional therapists in our in house rehab department
Located at 3 Upton Park, Rochester, NY 14607
Job Responsibilities :
- Provides skilled therapeutic intervention and services under the supervision of a Physical Therapist.
- Follows all regulations and guidelines of facility including compliance with confidentiality and HIPPA.
- Adheres to discussed scheduled hours and conducts self in a professional manner at all times.
- Provides documentation of patient’s condition, and provide caregiver / patient education as appropriate.
- Regularly discuss es with supervising Physical Therapist patient’s progress and any concerns or guidance needed .
- Ensures safe treatment practices and monitors patient ’ s engagement and responses to treatment , modifies as necessary .
