Job Summary:
As a Supervising Case Manager for Technology Pathfinder (MAPs), you will oversee and mentor a team of case managers supporting individuals with intellectual disabilities to achieve greater independence. You will ensure the effective implementation of remote technologies while fostering collaboration among families, circles of support, and staff. This role requires strong leadership skills and a commitment to compliance with organizational and regulatory standards.
Pay Rate:
$20-22
Monday - Friday, 8 am - 5 pm
Reporting Relationships:
Reports To: Director of Technology
Supervises: MAPs Case Managers
External Relationships: Innovation Coordinators, Participants’ Family Members/Guardians, Sister Agencies for Adults with Intellectual and Developmental Disabilities, State Department of Disability and Aging.
Supervisory Responsibilities:
- Team Leadership: Provide guidance, training, and mentorship to case managers, ensuring they effectively design and implement roadmaps to independence for individuals with intellectual disabilities.
- Performance Monitoring: Evaluate the performance of case managers regularly, offering constructive feedback and identifying areas for improvement.
- Compliance Oversight: Ensure all case managers adhere to Sertoma Center policies and procedures as well as standards set by Licensure, Quality Assurance, and the Center.
- Coordination: Facilitate collaboration among team members and external stakeholders to efficiently achieve program goals.
- Documentation Review: Monitor and approve all paperwork completed by case managers, including Milestone improvements and proofing, Monthly Service Reviews, and assessments.
Case Management Responsibilities:
- Collaboratively design roadmaps to independence for individuals with intellectual disabilities.
- Serve as a guide and mentor to achieve milestones towards independence.
- Work with families and circles of support to help individuals served achieve their goals for independence.
General Expectations of Performance:

- Set schedules with supported individuals and their circles of support.
Requirements
Requirements:
Education: High school diploma or equivalent required; bachelor’s degree preferred in social work, sociology, psychology, special education, or related field.
Experience: Minimum 6 months experience working with persons with disabilities required; supervisory experience preferred.
License/Registration: Valid TN driver’s license with a good driving record.
