Your Role
As the Residential Service Coordinator, you will be responsible for the development, implementation, and facilitation of the Person Centered Planning process for persons served. Residential Service Coordinators ensure integration of person centered planning and implementation strategies through the involvement of the individual, family, guardian, other service providers, agency staff, and others identified by the individual.

Your Role
As the Residential Service Coordinator, you will be responsible for the development, implementation, and facilitation of the Person Centered Planning process for persons served. Residential Service Coordinators ensure integration of person centered planning and implementation strategies through the involvement of the individual, family, guardian, other service providers, agency staff, and others identified by the individual.
Responsibilities
- Responsible for the development, implementation and facilitation of implementation strategies for assigned individuals, ensuring integration of habilitation planning and provision of services through person centered planning.
- Monitor and modify implementation strategies in such a manner as to ensure that services outlined continue to meet individuals’ needs and reflect the priorities of the individual served.
- Participate in the Intake process for screening, program placement and appropriate movement of individuals served.
- Provide staff with direction, training, and support needed for PCP implementation, data collection, and provide follow-up on PCP/Implementation strategy revisions. Work cooperatively with staff to ensure appropriate access to services as identified during the PCP process.
- Ensure individuals receive annual physical, dental and follow-up appointments in a timely manner and in accordance with Rule 115. Accompany individuals to initial specialty medical appointments and other appointments as needed. Communicate any needed medical information to RN, guardian, or other family members.
- Maintain 24-hour on-call cell phone accessibility to respond to medical, behavioral, and clinical emergencies regarding residential consumers.
- Ensure the delivery and/or coordination of all CILA services in compliance with DHS Rule 116, CARF standards, agency mission, agency policy & procedure, program guidelines and best practice.
- Ensure persons served have appropriate means of identification, including but not limited to, state ID, Birth Certificate, social security card, etc.
Qualifications
- Bachelor’s degree in human services field of study required (Alternative Bachelor’s Degree with 30 semester hours in human services areas approved by DHS for QIDP Certification).
- Certification by DHSA as a qualified QID, or the ability to complete training and obtain certification within 6 months.
- Minimum of one year experience working with individuals with developmental disabilities and/or mental illness, preferred, or the ability to reach one year of working experience within 6 months.
- Prior experience in a residential setting preferred.
- Effective communication skills, both oral and written.
- Valid driver’s license and documentation of current auto insurance, with an acceptable motor vehicle report.
- Proficient in the use of computers, software applications, and working knowledge of Microsoft Office Suite programs.
