Job Description:
As a Service Coordinator, you will be responsible for providing support to individuals with intellectual disabilities and/or developmental disabilities. Your primary duties will include:
- Routinely exploring options, areas of interest and preferences and growth opportunities for each person served
- Establishing and monitoring an annual Person-Centered Plan that identifies outcomes important to the person, outlines support needed and required, and secures ongoing support services
- Identifying and securing ongoing support services and providing assistance to individuals with developmental disabilities
Requirements:
To be successful in this role, you will need to possess:
- A bachelor's degree from an accredited education program in a human services field or a non-human services field with two years of experience in a human services field
- Experience working with people with developmental disabilities
- Strong communication and interpersonal skills
- Ability to adapt to changing situations and priorities
