Service Management Group is seeking a Clinical Consultant to join our team! In this role, you will provide community based clinical direction to clients offering personalized care both in their home and virtually. Building deep meaningful relationships with your clients, you will use your expertise and our innovative technology to serve as a coach, educator, and guide on their health journey.
This position combines clinical nursing with cutting-edge technology to not only address medical needs but also empower clients to take control of their health. Requirements:- Associate Degree or Bachelor's Degree in Nursing.
- Current, unrestricted MN State RN License
- BLS Certification
- At least 2 years nursing experience in the following areas: home care, mental health, community-based nursing, skilled nursing facility, public health, visiting nursing, and/or private duty nursing (PDN).
- At least 1 year experience working with underserved populations such as those on the Medicaid waiver program or other socio-economically challenged populations.
- Ability to remain calm and composed under pressure, ability to troubleshoot in unexpected situations.
- Comfortable working in client’s homes and with clients and caregivers with multiple, complex and chronic health conditions including behavioral health.
- Computer proficiency (Microsoft Office Suite, Google Suite, etc.) and EHR/EMR experience.
- Ability to learn new technology quickly and proficiently.
- Interdisciplinary team experience and ability to collaborate and communicate with all members of the healthcare team (Homecare nurse, IHS, PCA, case manager, healthcare specialties).
- Excellent oral, professional presentation, written skills are a needed plus must have the ability to work independently 90% of the time.
- Must be able to organize your own workflow of daily tasks and be able to complete them in an efficient time.
- Must pass a background study with the Minnesota Department of Human Services and negative Tuberculosis (TB) test.
- Have a valid driver’s license, clean driving record and reliable transportation to use for work driving purposes.
