Locations: Bronx and Manhattan
Schedule: Mon-Fri 9a-5p split position Bronx 3 days, Manhattan 2 days
SCOPE OF ROLE:The Licensed Practical Nurse will work as part of a care team of mental health professionals assisting clients in a permanent housing / Mixed Use Living environment transitioning from homelessness to housing in the community. The team will follow individuals into the community using the evidencebased Self – Wellness Recovery model of care to ensure that they become both linked and engaged with identified community providers and that their support network is sufficient to meet their needs. The LPN is responsible for the diagnosis and treatment of human responses to actual or potential health problems through such services as medical case management, referrals, health teaching (this might include group education) and counseling. The nurse will provide interventions supportive to or restorative of life and well-being; and execute medical regimens prescribed by a physician, PNP or another licensed health care provider.
ESSENTIAL DUTIES & RESPONSIBILITIES:• Complete medical assessments and Communicable Disease Risk Assessments within mandated timeframes• Coordinate delivery and reception of medications with pharmacy• Track all medications received and administered as well as coordinate return of unused medicationsto pharmacy• Observe self-administration of medications for detoxification according to established protocols• Refer for required medical services• Carry out urine screenings in accordance with program policy• Provide education and consultation to individual clients and/or groups on their health needs• Assess clients’ medical record and coordinate medical care with medical director and/or outsideclinics• Assure clients’ compliance with prescribe medical regimen• Complete timely and accurate documentation on medical services rendered • Other duties as assigned
Salary: $65,000-70,000/year
Qualifications
REQUIRED EDUCATION AND EXPERIENCE• New York State Licensed Practical Nurse• Good written and oral communication skills required• An ability to work independently as well as part of an interdisciplinary team
PREFERRED QUALIFICATIONS & SKILLS• Experience in direct clinical work with psychiatrically disabled populations preferred• Bilingual English/Spanish speaking preferred
