Position Overview :
Title : Outreach Specialist
Department : Treatment Services
Reports to : Clinical Supervisor
Essential Job Functions :
- Track and Monitor all Referral and Intakes
- Adhere to agency standards of required documentation including assessments, consents, screening tools, progress notes and any additional intake documentation
- Participate in staff meetings, case conferences, utilization review, staff development, clinical supervision and trainings
- Assess all new admissions for Medicaid eligibility while maintaining compliance with billing targets and Medicaid documentation
- Identification of any problems in daily living skills; hands on assistance and / or referral in addressing problems in daily living skills.
- Liaison to community based services and groups including civic, social service government, and business organizations.
- Other duties as assigned by supervisor
- Travel may be required.
