Serving Children and Reaching Families, LLC is looking for a highly motivated individual to join our team as a Mental Health Targeted Case Manager. The ideal candidate will have a passion for helping others and will be able to provide compassionate care in a professional environment.
As a Mental Health Targeted Case Manager, you play a direct role in helping the lives of our clients each day by providing and promoting an environment of long-term recovery.
Position Summary:
Applicant for this position is a 1099 Independent Contractor who provides Outpatient Mental Health services to adults and adolescents. Duties include assessing, monitoring, advocating on behalf of, and linking clients to community resources. Services are provided to clients who meet Medicaid or state criteria for case management.

Required Knowledge and Abilities:
- Applicant must have applicable knowledge of Medicaid rules for Targeted Case Management service delivery when providing services to Medicaid enrolled clients
- Must have knowledge of and apply recovery-based principles and practices in service delivery that includes addressing and documenting the client’s needs, strengths, abilities, barriers and preferences
- Must be knowledgeable of community resources
- Must be able to work with individuals who have a mental health disability
- Applicant must have the CBHCM certification provided by the Florida Certification Board
- Must be reliable and a team player
- Ability to provide services in the client's home, or in other community settings
- Ability to attend virtual team meetings, supervision meetings, and trainings according to agency needs
- Applicant must have equipment to perform job duties and a stable internet connection
- Has knowledge of psychiatric diagnosis and ability to recognize signs and/or symptoms of psychiatric illness.

Qualifications and Education Requirements:
To be eligible as a Mental Health Targeted Case Manager at Serving Children and Reaching Families, LLC, applicant must meet the following minimum criteria:
- Bachelor’s Degree in Social Work or a Human Service-related field from an accredited college or university;
- One year of Full-Time or equivalen experience providing targeted case management services to adults experiencing serious mental illness and/or children with emotional disturbances;
- Has completed the CBHCM certification provided by the Florida Certification Board or currently holds a provisional certification.
- Has knowledge of available resources within service area(s);
Additional Information:
- Applicant must successfully pass all Pre-Employment Screenings including but not limited to a Federal II Background check, a Local Background check, a Pre-Employment Drug Screening, and applicable licenses and insurances
- Must have a valid Driver’s License and Motor Vehicle Insurance and Registration;
- Bilingual (English/Spanish) or French-Creole a bonus!
