What are the responsibilities and job description for the Caregiver - Hospital Discharge position at Serving Spirit Home Care?
Serving Spirit is seeking HHA's, CNA's, and Caregivers to help support those during the hospital discharge process. You will assist those being discharged from the hospital to their homes to ensure that they are setup safely at home and provide any immediate care.

Personal Caregivers and CNA's or Home Health Aides are needed for DAY and EVENING!
Summary
As a Personal Care Assistant, you will provide essential support to individuals in need of assistance with daily living activities, including bathing and personal hygiene. Your premium skills in patient observation, first aid, and home care will enhance the quality of care provided. Join us in making a meaningful impact on the lives of others.
Responsibilities
- Provide companionship and emotional support to enhance clients' well-being.
- Monitor and document patient observations, reporting any changes in condition.
- Maintain cleanliness and organization in living spaces.
- Prepare meals and assist with nutritional needs as required.
- Collaborate with healthcare professionals to ensure comprehensive care.
Qualifications
- Proven experience in caregiving and patient care, preferably in a CNA role
- Strong skills in patient observation and assisting with activities of daily living (ADLs)
- Knowledge of first aid and emergency response procedures
- Experience in home and community care, as well as group home settings
- Ability to maintain cleanliness and organization in living spaces
- Familiarity with long-term care and social work principles
