Responsibilities
- Facilitate and support participants in creating individualized, person-centered plans.
- Guide participants in understanding and exercising their right to make informed choices, including self-direction.
- Assist with Medicaid renewals and other eligibility processes to ensure uninterrupted services.
- Provide timely, effective communication, and respond promptly to participant needs.
- Ensure participants are aware of their rights, including the right to be free from abuse, neglect, and exploitation.
- Monitor and reassess participant plans to align with evolving needs and preferences.
- Collaborate with participants, their support networks, and service providers to coordinate effective, non-duplicative services.
Qualifications
- Education: Bachelor’s degree required.
- Experience: 3–5 years of related experience.
- Skills: Strong communication, time management, assessment, and person-centered planning skills.
- Knowledge: Understanding of community resources, rights and dignity, and person-centered services.
- Other Requirements: Valid driver’s license and ability to drive.
