Overview

The Case Manager is a vital member of a multi-disciplinary team that works to improve mental health functioning of youth in residential programs at YOU, Inc. The Case Manager will be responsible for providing direct care and case management services including service coordination, resource development, and supporting treatment plan implantation and aftercare follow-up, as well as working in ratio as needed depending on census. The Case Manager should attend to risk issues, documentation of care, timeliness of documentation, professionalism, caseload expectations, and administrative issues related to providing services.

Responsibilities

• The Residential Case Manager will provide support and case management to youth in an acute or congregate care residential placement. Case Managers will work to ensure that the goals of a safe and stable discharge are met through: o Supporting the implementation of service plans, o Connecting to resources and natural supports, o Participating in team meetings as needed, o Coordinating and following-up on aftercare of discharged clients o Communicating pertinent information effectively and in a timely manner,o Responding to crises by obtaining appropriate intervention and services as appropriate.

• The Residential Case Manager will facilitate client care through being an active member of the client’s multi-disciplinary team (includes psychiatry, social work, clinical counseling, and direct care/Residential counseling); coordinating with the family/caregiving system for the client; coordinating care with external providers (PCP, medical specialists, etc. assuming there is the legal right to do so); following-up with clients post discharge; and, as directed, acting as liaison between treatment team, referral source, community agencies, court officials, outside therapists, DCF or other state agencies, and other entities as appropriate (assuming there is the legal right to do so).• Work in ratio in the milieu in any capacity needed for programmatic success. The Case Manager may be mandated to work in ratio. To this end, the Case Manager role requires training and ongoing refresher training in:o CPI/De-escalation and Physical Management o First Aid, CPR, and use of Universal Precaution o Medication Management and/or MAP o Van Safety/Drivers Safety

• Case Managers will provide the highest-quality care to clients in keeping with best-practice, agency standards, and the directives of the position. The Case Manager will practice within ethical guidelines; maintain privacy and HIPAA standards. • Document all services in the required data entry system (e.g. agency Electronic Health Record, Virtual Gateway, CRM), in keeping with established timeframes, best practices, and through required forms/documents. • Monitor and take corrective action to mitigate safety risks in the milieu, and report issues of immediate concern to the Program Director, or the next level manager.

• Provide services such that the following values are embedded in practice:o Promote client-centered care that adheres to best practice standards and recognizes “client voice and client choice,” o Recognize the multiple perspectives of clients and their caregiving systemo Be cognizant of issues of inclusion, equity and diversity of people that might otherwise be marginalized or oppressed.o Utilize active and reflective listening as well as the exchange of feedback,o Orient oneself in a strengths-based approach,o Develop an understanding of the impact of trauma and substance abuse,o Provide positive regard, empathy, curiosity, respect, and professionalism, o Establish and maintain clear expectations and boundaries, ando Remain cognizant of and proactive about any issues of safety / risk.• Attend and participate in a variety of meetings and trainings including but not limited to: supervision, rounds, case conferences, program team meetings, trainings, and other staff meetings as determined. • Engage in ongoing learning and development of skills and professional growth through trainings, supervision, and ongoing delivery of services.

Qualifications

• Knowledge of youth residential treatment, factors of a therapeutic milieu, and of the child behavioral health/child welfare system.

• Developing and/or demonstrated proficiency in individual/youth and family case management and/or wrap-around care. Developing and/or demonstrated proficiency in providing care planning, care coordination, service delivery resource development, advocacy, and/or discharge/after-care planning.• Proof of a valid driver’s license, acceptable driving record, and reliable personal transportation to fulfill essential job functions is required. Privately owned vehicle has current and valid state registration and safety inspection sticker, is safe, and is ensured according to the minimum state requirements (pertaining to bodily injury and liability). As per Massachusetts state law, driving under the influence of any substance is against the law, prohibited, and may be grounds for immediate termination.• Demonstrated and/or developing competency with establishing a therapeutic rapport with consumers and effective collaborative relationships with providers. • Demonstrated ability to exercise discretion, confidentiality, collaboration, ethics, appropriate boundaries and good judgment. Demonstrated ability to seek out supervision and consultation as needed. • Thorough, clear, and timely oral and written communication, strong organization skills, timeliness, and strong ability to engage and work effectively with others.

Physical Requirements

• Must be able to perform sedentary aspects of position, including, but not limited to desk and computer work, typing, driving to meetings and trainings, as well as driving/transporting clients as necessary.• Must also be able to perform therapeutic activities with youth.• Must be in relatively good health and be able to perform the duties listed above.• Employee may be required to intervene in order to de-escalate and/or provide safety and if essential, provide physical management of clients using methods and techniques as trained and approved by YOU, Inc.
