Job Title: Licensed Practical Nurse, Medsurg/Tele (LPN)

Location: Rochester, NY

Blended Rate: $1260/wk

Shift: 3x12s - 36hrs - Days/Nights rotating

Job Summary:

We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our Med-Surg/Telemetry team in Rochester, NY. In this role, you will provide essential nursing care to patients recovering from surgeries, chronic illnesses, or other acute medical conditions. You will monitor vital signs, assist with medication administration, and respond promptly to telemetry readings, ensuring high-quality care in a fast-paced environment.

Key Responsibilities:

• Provide direct patient care, including monitoring vital signs, administering medications, and assisting with wound care and daily activities.
• Monitor telemetry equipment and report abnormal cardiac rhythms or changes to the care team immediately.
• Collaborate with RNs, physicians, and other healthcare providers to develop and implement individualized patient care plans.
• Assist with admission, discharge, and transfer processes, ensuring smooth patient transitions.
• Document patient care, assessments, and interventions accurately in the electronic medical record (EMR).
• Educate patients and their families on post-hospital care, medication instructions, and lifestyle modifications.
• Respond promptly to patient call lights and provide emotional support to patients and their families.
• Ensure patient safety by adhering to infection control protocols, medication safety standards, and hospital policies.

Qualifications:

• Current and unrestricted LPN license in the state of New York.
• Previous experience in a Med-Surg or Telemetry unit preferred.
• Basic Life Support (BLS) certification required; Advanced Cardiac Life Support (ACLS) is a plus.
• Proficient in telemetry monitoring and understanding cardiac rhythms is preferred.
• Strong organizational, communication, and multitasking skills.
• Ability to work effectively under pressure and in collaboration with a multidisciplinary healthcare team.
