As Case Coordinator, you will be responsible for providing organizational assistance and program support, including admissions procedures, direct participant care, and coordination of internal and external services.Responsibilities : Develop individual programs and assure that the program meets the participant’s needs, optimizing the participant’s potential to be integrated into the community; ensure the provision of appropriate services to designated program participantsAssist designated program participants, as needed, to understand the overall goals and processes being implemented in the programsAct as a liaison for families and provide them with information and resources as requestedOrganize and chair program participant conferencesReview the designated program participant's reports (evaluation, progress, and discharge) for consistency and coordination of goals; ensure the appropriate documentation / recordkeeping of designated program participantsResponsible for development, coordination, implementation, and follow-up of discharge planning as directedAssist in internal communication to team members regarding case-specific issuesAttend required meetings, including clinical team meetings and regularly scheduled supervision meetingsReview monthly bills for designated program participants after receipt from accounting staff; identify any discrepancies to accounting staffCoordinate utilization with the Case Manager relative to program participant’s incentive accountOrganize and facilitate internal medical appointments as requested by management, overseeing timely completion of annual medical appointmentsEnsure the appropriate documentation (including summaries and tracking information) of designated program participants for internal medical appointments and distribute as requiredDirect, develop, coordinate, implement, and follow up on healthcare planning as directed by the Program Director / Manager; ensure appropriate medical follow-up for designated program participantsCommunicate with community resources and persons outside of the program to assure quality program participant healthcare for all assigned program participantsAssist in training new staff involved with overseeing medical care as requestedQualifications : Bachelor's Degree in Social Service-related field RequiredTwo years of direct or indirect support services to persons with disabilities in a medical and / or rehabilitation settingStrong attention to detail, organizational skills, and the ability to multi-task to meet deadlinesEffective communication skills to manage relationshipsSelf-motivated and collaborative; a team playerA reliable, responsible attitude and a compassionate approachCommitment to the company’s mission and valuesSpanish-BilingualWhy Join Us?Competitive Pay : Compensation and full benefits package for full time employees, including a 401(k) with a discretionary match potentialMedical, Dental and Vision Insurance plus 401(k) Retirement PlanPaid Time Off (PTO) – Start accruing on your first day of employmentPaid Holidays for full time employeesWe invest in your professional growth through continuing education, training, and moreSupportive relationships with coworkers and supervisors who help you grow and learnReferral Program - Earn $$ for each applicant you refer to the company!Employee Discounts / “Perks At Work” – Discount program available to employee’s, offering online access to more than 30,000 discounts and exclusive offers through Sevita’s vendor relationshipsWe have meaningful work for you – come join our team – Apply Today!Sevita is a leading provider of home and community-based specialized health care.
