Job Description
As a Case Manager at Sevita, you will play a vital role in supporting individuals with intellectual and developmental disabilities to achieve their goals and maximize their ability to live outside an institution. Your responsibilities will include:
- Providing case management services including intake, assessment, crisis intervention, advocacy, referral, and monitoring of families.
- Gathering social history to assess family strengths and weaknesses, and existing problems.
- Conducting home visits and office-based services, and accompanying individuals being served.
- Coordinating care with outreach workers, health educators, nutritionists, and other service providers to prevent service duplication and ensure high-quality care.
- Identifying and procuring group services appropriate for the individual to maximize their ability to live independently.
- Planning and assisting in obtaining services from third-party service providers.
- Developing a plan of care for each individual to include services such as advocacy/case management, independent living skills, training, peer support, physical therapy, cognitive therapy, etc.
Required Skills and Qualifications
To be successful in this role, you will need to possess:
- A Bachelor's degree in human services or equivalent education and experience.
- One year of related work experience and knowledge of case management.
- A valid driver's license, registration, and insurance.
- Current CPR/First Aid Certification as required by state/program.
- Ability to establish working relationships with individuals served.
- Demonstrated competence in verbal and written communication skills.
- Ability to handle crisis situations.
