As we continue to expand our team, we are seeking a skilled and dedicated Licensed Practical Nurse to join us in providing high-quality home care services.
Essential Job Functions of a Home Health Care Nurse:
- Perform home health nursing duties in patients homes.
- Administer medications, perform wound care, and provide other skilled nursing services in accordance with physician orders.
- Review consumer medical documentation and or health referral forms, as relevant to the case, and adjust care plans are needed.
- Health promotion, teaching, and training of family members
- Understands and demonstrates commitment to the company's mission through daily communication with participants, families, community, and staff.
Certificates, Licenses, and Registrations:
- Current Massachusetts LPN Licensure
- All state required training and certification completed in mandated timeframes.
- Driver’s License / Reliable means of transportation
Qualifications:
- 1 year- experience in Home Health Nursing.
- Good Physical Assessment skills.
- Must be able to use and understand electronic mobile devices for documentation and other electronic applications for the job
- Effective Time Management
- Travel Required
