Senior Cognitive Retraining Specialist - Home and Community FLEX hours and make your own schedule
As a Senior Cognitive Retraining Specialist, you will play an important role in helping us achieve our mission of providing quality services to the individuals we serve.
- Coordinate services provided to individuals served, assess the continuing need for cognitive therapeutic services at an individual's current level of care, and ensure quality of care delivered.
- Provide case management and counseling services to individuals served in-home and community-based settings.
- Assess referred individuals and lead development of service plan in conjunction with treatment team and funding sources in accordance with corporate, contractual, and regulatory standards.
- Create, coordinate, and evaluate service plans through partnerships with stakeholders based on assessed needs.
- Provide pre-service introduction and in-service continuing education to mentors as appropriate per regulatory requirements.
- Provide ongoing monitoring and consultation to mentors regarding work with individuals and implementation of service plan.
- Conduct mentor and family home visits and facilitate mentor group meetings as required.
- Maintain current and complete case and reporting records in accordance with corporate and regulatory standards.
- Assist with all legal, licensing, regulatory, and certification activities as needed.
- Participate in rotational on-call system and assist with emergency coverage and crisis intervention support as needed.
- Participate in all activities designed to enhance and build an active social network of mentors.
Qualifications:
- Bachelor's degree in human services; Master's degree preferred.
- Three years of experience in the field working with individuals with developmental disabilities or mental health issues.
- Licensure and/or certification in field required as determined by site contractual agreement and state standards.
- Current driver's license in good standing, car registration, and auto insurance if providing transportation for individuals receiving services.
- Self-motivated and detail-oriented with ability to multi-task.
- Strong analytical skills with the ability to collect information from different sources.
