ID# 36153A - POSITION DESCRIPTION : Initial assessment and referral of homeless, mentally ill clients. Provide individual and group counseling / case management and crisis intervention as necessary. Facilitate groups.
Provide direct and indirect client services.
Participate in assertive assessment, evaluation and referral of homeless dual diagnosis clients. Assess client’s level psychosocial impairment, substance abuse history, self-care potential, family and support network availability, independent living needs, financial status, education, employment or training needs, and social / recreational needs. Refer those appropriate for intake at Cornerstone facility.
Complete all documentation within mandated time frame and in compliance with county, state, and Center guidelines.
Provide crisis intervention – intervene with the client / others at the onset of a crisis to provide support and assistance in problem resolution and to coordinate or arrange for the provision of other needed services.
Complete all agency paperwork and client documentation on a daily basis or as required.
Attend Cornerstone meetings as part of a multi-disciplinary team.
Facilitate motivational groups; substance abuse, coping skills, conflict resolution.
Will perform other duties as assigned.
