The Care Manager (Acute Care) I (CM I) role must optimize and integrate the interface between clinical and financial systems supporting positive health outcomes.
Responsible for :
- The assessment, integration, and facilitation of patient discharge plans focusing on the coordination of care for the medium to high-risk patients across the healthcare continuum.
- Goal is for discharged patients to possess a “reasonably safe discharge plan”, engaging support systems if patient allows and are available.
- Ensures the delivery of cost-effective, outcome-oriented and quality of care within an expected length of stay.
- Manage patient care to prevent fragmentation and duplication of services.
- Works in collaborative practice with the physician and other members of the health care team to meet patient-specific and age-related needs linking cost and quality to patient care.
- Role ensures the care is delivered at the “right” level of care and setting.
Salary : $32 - $59
