What are the responsibilities and job description for the CVP Case Manager Cicero position at SGA YOUTH & FAMILY SERVICES NFP?
Description
- Create assessments for victims of violence.
- Assist clients in applying for the Illinois Crime Victims Compensation Program.
- Coordinate with CVP Responder on appropriate services and follow up within 72 hours of incident
- Collect resources on recreational activities, housing, food, clothing, school programs, vocational opportunities or services, providers to teach life skills, and relevant mental health services.
- Assists clients in developing natural resources and contact in social support networks.
- Provide case management services through one-on-one meetings with families to determine and meet their needs.
- Interpretations may be needed to assist families in applying for public benefits and services.
- Verbal or electronic translations of brochures, forms, and application instructions, as required, to access services
- Review to ensure all eligibility requirements are met, including uploading required documentation.
- Conduct community education about public benefits available to immigrants in IL. Outreach to potential eligible participants and their landlords.
- Establish MOUs with community partners to create a referral/linkage system for clients enrolled in the program.
Requirements
- High School or GED. Associates preferred.
- At least two years’ experience working with youth and families
- Must be committed to working with young people and their families who are affected by community violence
- Lived experience in the community
- Flexible work schedule including weekends and holidays
Salary : $26 - $32
