Part Time .5 FTE
SGA provides a wide range of social services in some of Chicago's most challenged and consistently underserved communities and creates long-term and community-wide change.
MAJOR DUTIES AND RESPONSIBILITES
- Develop and maintains meaningful, productive relationships with families.
- Conduct periodic home visits and phone calls throughout the year to assess family resources and needs.
- Identify services currently available and those needed by families enrolled in the Early Head Start program.
- Develop relationships with other community-based service providers targeting parenting families.
- Refer and link children and parents to needed services including but not limited to (EI/LEA, WIC, housing, food pantries, medical clinics, etc.)
- Work in collaboration with other SGA staff and consultants to monitor, track and coordinate services for children and parents.
- Maintain records and documents services in a timely manner.
- Facilitate parent’s training and promote parents’ overall involvement and participation in the trainings.
- Assist in planning and implementing parent activities during group socializations and encourage parent participation in program activities and obtain feedback from parents regarding the level of satisfaction with the services they receive.
- Attend regularly scheduled staff meetings, family support meetings, social service and health meetings.
- Contribute to the Agency’s efforts related to prevention and integrated service delivery with other community providers.
- Support and contribute to inter-system collaboration with other service providers to improve social service delivery systems.
- Collaborate with Education staff to facilitate children’s transitions.
- Collaborate with Disabilities Agencies to recruit children with disabilities into the program.
- Complete other duties as assigned.
Requirements
Education:
- AA, AAS, BA, Advanced degree or credential or certification in Social Work, Family Services, Human Services, Counseling or in related field; or expected completion within 18 months of hire.
Experience:
- Experience working with young youth is required.
Other:
- Sensitivity to cultural diversity is required. Ability to communicate and cooperate with diverse families, various professionals and community groups.
- Must be able to make home visits to present and prospective and former clients in the clients’ own home.
- Ability to write routine reports and correspondence.
- Ability to speak effectively before groups of customers or employees or the organization.
- Exercises discretion and independent judgment in executing tasks.
- Must demonstrate the ability to establish helping relationships with families of varying educational, experiential, and socio-economic backgrounds and handle emergency and crisis situations.
KEY COMPETENCIES:
- Competent in Microsoft Office tools and inputting services in an online database.
- Schedule may be flexible and may include some evening and weekends.
- Valid Driver’s License and Automobile Insurance.
- Reliable transportation.
PHYSICAL REQUIREMENTS:
- Must be able to remain stationary 50% of the time.
- Must be able to frequently move inside the office to access file cabinets, office machinery, etc.
- Must be able to transport oneself to and from home visits.
- Occasionally ascends/descends stairs
- Constantly positions oneself to assist children during activities.
- Frequently moves desks, chairs, tables, easels, and children's toys.
- At times may be required to lift up to 25 lbs. for various classrooms and events needs.
- Frequently moves in outdoor weather conditions.
