This role is responsible for providing case management services, coordinating support, and facilitating communication between clients and the program team.
Primary Duties And Responsibilities
- Act as the primary contact for participants and hotel/motel staff, ensuring continuous accessibility via the designated office phone.
- Maintain up-to-date knowledge of client needs, ensuring seamless transitions in coverage when unavailable.
- Collaborate with the Behavioral Health Counselor and Senior Program Director to ensure effective backup coverage during absences.
- Participate in regular supervision and team meetings to discuss client status and anticipated needs.
- Respond promptly to emergencies, coordinating with the team to ensure appropriate coverage and intervention.
- Forward calls and relevant information to backup staff when necessary.
- Document all client interactions, case updates, and emergency situations accurately and in a timely manner.
- Provide ongoing support and referrals to participants as needed
- Maintain a flexible schedule including weekends and holidays
Education:
- Bachelor’s degree in social work, psychology, human services, or a related field (preferred).
- Bachelor’s degree in social work, psychology, human services, or a related field (preferred).
- Experience in case management, social services, or behavioral health.
- Strong crisis management and problem-solving skills.
- Excellent communication and organizational abilities.
- Ability to work flexible hours, including being on-call as required.
- Knowledge of domestic violence, LGBTQIA , and human trafficking, community resources, and support services.
- Sensitivity to cultural diversity is required. Ability to communicate and cooperate with diverse families, various professionals and community groups..
- Ability to write routine reports and correspondence.
- Ability to speak effectively before groups of customers or employees or the organization.
- Exercises discretion and independent judgment in executing tasks
- Must demonstrate the ability to establish helping relationships with families of varying educational, experiential and socio-economic backgrounds and handle emergency and crisis situations.
- Schedule may be flexible and may include some evening and weekends.
- Valid Driver’s License and Automobile Insurance.
- Bilingual/bicultural in Spanish/English, strongly preferred
- Must be able to remain stationary 50% of the time
- Must be able to frequently move inside the office to access file cabinets, office machinery, etc.
- Must be able to transport oneself to and from home visits.
- Occasionally ascends/descends stairs.
- Constantly moves to assist during program activities.
- At times may be required to lift up to 25 lbs. for various classrooms and event’s needs.
- Frequently moves in outdoor weather conditions
