CRS Case Manager - Portland Area
Shalom House, Inc (SHI) is the largest provider of community residential programs and housing for adults with severe and persistent mental illness in Maine. Join a Community Rehabilitation Service (CRS) Team of case managers and daily living support specialists who provide services to adults with severe and persistent mental illness living in the community. The CRS case manager will work to help coordinate and collaborate with other providers to ensure that our client’s housing, medical, behavioral health, and community needs are being addressed. This will be done through participation in an interdisciplinary team model to develop Treatment Plans with clients and provide a variety of services and support to assist them in meeting their identified goals. Caseloads are small at 11-15 clients per case manager, all clients live in the Portland area, there are plenty of free training opportunities and a supportive, collaborative team. CRMA training will be provided.
Associate degree in social services or related field with 1 year of successful community-based mental health work preferred. Degree requirement may be substituted for additional experience if have MHRTC. Must be MHRT-C certified or provisional eligible.
Applicants must have a valid Maine driver’s license with acceptable driving history. We do criminal background checks and driving history checks on all potential new hires.
