Manager - SMM

The Care Manager Supervisor works to lead and support Care Managers at Shannon. The Care Manager Supervisor supports the development of patient-centered, team-based care. Responsibilities include oversight of day-to-day duties of Care Managers. S/he will supervise the support of primary care physicians (PCPs) and practices in managing their panel of patients. The Care Manager Supervisor ensures Care Managers are gathering and organizing patient data, identifying & addressing patient unmet needs, engage patients in their own care, gather summary information for treatment interventions, and enhance ongoing communication between the patient and her/his care team. The goal of the Care Manager is to facilitate high-value, patient-centered care that improves timely access to and provision of preventive services and chronic disease treatment. The Care Manager Supervisor also leads projects associated with population health, shared savings plans, and community-based health initiatives as needed.
