What are the responsibilities and job description for the DSP (Direct Support Professional)/ Caregiver position at Share Care USA?
*DSW to work with a male individual in his elder years who suffers TBI and some physical impairments but still ambulatory, although he is a fall risk.
* Days of the week depends on DSW’s availability but is required to work at least every other weekend. Weekend hours are Day shift 7am-7pm and night shift 7pm-7am.
PREFERRABLY SOMEONE TO WORK 3P-8AM, OR 8AM-7PM OR 7PM-8AM
* Following the Individualized Service Plan (ISP) of recipient as requested
* Housekeeping, Laundry, Cooking, managing of the groceries needed as well as household supplies, following structured schedule for his Activities of daily living, Ensuring a safe and clean environment.
* Some Community outings as long as he is physically able to. Wheelchair can be used for these. MUST BE OPEN TO ATTENDING CATHOLIC MASS with him, MUST be able to drive him to most of his appointments in Lafayette, or to his part-time job in Lafayette.
* Upbeat personality who is able to encourage positive behaviors.
* He is a smoker but does it outside and he limits his cigarettes to a few a day.
* Encourage and allow his independence in a safe manner as long as possible.
* Arriving on time at recipient’s home, (Have reliable electronic devices that can be used for EVV on every shift work)
* Completing time sheets and progress notes accurately and on time, along with maintaining accurate and concise records
* Must be able to drive to arrive at work and to transport client in the community, along with all up-to-date required documentation (Current Drivers License, Current Car Insurance, Current Registration and Current Inspection Sticker).
