Direct Support Professional (DSP)

Role and Responsibilities
- Modeling and teaching of skills to increase and promote independence.
- Assure that the individual’s goals, ISP’s, and behavior plans are implemented as written. Follow prescribed recommended treatments.
- Effectively communicate progress, limitations, obstacles or concerns with team members including Program Specialist, family, or Supports Coordinator.
- Ensure proper supervision is provided as outlined in the ISP; for home, and community.
- Attend all required meetings for the individual. Be an active member of the team and provide information and insight at quarterly reviews or other team meetings.
- Record and report all required data to the Program Specialist.
- Take appropriate steps in the recording and reporting of emergencies and or incidents.
- Report any incidents immediately to supervisor if Monday through Friday 9am-5pm or on-call supervisor after hours, weekends, or holidays.
- Complete daily shift notes, along with all behavioral data if applicable.
- Follow schedule as designated by Program Specialist and other team members.
- Attend all required trainings.
- Provide transportation and support while in the community.
- Implement outcomes as established in ISP
- Follow proper call out procedures as established in orientation
