25.310 - $31.640 - $37.970

What You Will Do
Provides support to administrative team by coordinating Population Health priorities. Ensures effective and efficient systems. Ensures processes are utilized to achieve and improve patient outreach, care and operational outcomes. Supports and facilitates care coordination activities and services provided by RN Case Managers and assists with oversight of Care at Home activities and initiatives. Collaborates and assists with strategies that target care management engagement and growth.
Required Qualifications
- H.S. Diploma or Equivalent
- 2 Years experience with project coordination or management activities for an office or health care organization.

Essential Functions
Participates as a member of the department team as related to achievement of enterprise, entity, and department goals and targets.
Administers, tracks, and records surveys to patients and other customers as to meet department guidelines and goals.
Communicates and collaborates with Population Health customers.
Proactively identifies issues or concerns affecting good working relationships and applies problem solving techniques.
Brings issues to supervisor for discussion in a timely manner and accurately reports pertinent information.
Assists Case Managers with assigned patient follow-up activities, achieving outreach interval targets per department guidelines.
Schedules appointments in a broad range of situations, arranges related resources, and confirms participants.
Prepares meeting agenda and attachments, may attend, participate, and record minutes for meetings as directed.
Generates assigned reports, ensures readiness of all documents and timeliness of information.
Coordinates vendor activities.
Manages vendor accounts, ensures invoices are accurate and prepares for signature.
Responds to urgent and emergent situations in a calm and capable manner.
Utilizes good judgment and problem solving ability.
Participates in and prepares for site inspections and inventories.
Researches and gathers data from various sources and generates special projects / reports / summaries.
Report generation may involve analyzing statistics and preparing spreadsheets, databases and other documents.
Proficient in the use of computer applications i.e. Word, Excel and PowerPoint.
Composes correspondence, memos, presentations, flyers, charts and other documents as directed.
Assists with orientation of new staff.
Facilitates inter-departmental cooperation and teamwork.
Acts as a resource and role model for staff.
Demonstrates professional conduct, appearance and communication.
Respects diversity, acts as a positive change agent, utilizes ethical decision making, critical thinking and problem solving skills.
Manages special projects as directed.
Works closely with leadership to launch new projects relating to population health, patient outreach, patient engagement, collaboration with outside departments and telehealth activities.
Works closely with Population Health manager and leads to coordinate projects and research outcome information for the Care Management teams.
Works closely with Case Managers to support care coordination activities through assigned tasks and patient outreach and communication while adhering to prescribed turn around times and department productivity guidelines.
Develops proactive working relationships with clinic staff, physicians, and vendors and communicates information as needed.
Initiates establishment and maintenance of record keeping systems to ensure efficient retrieval of documents.

Salary : $37,970
