Hourly Pay Range (Minimum - Midpoint - Maximum) :
41.432 - $53.460 - $65.489
The stated pay scale reflects the range that Sharp reasonably expects to pay for this position. The actual pay rate and pay grade for this position will be dependent on a variety of factors, including an applicant’s years of experience, unique skills and abilities, education, alignment with similar internal candidates, marketplace factors, other requirements for the position, and employer business practices.
What You Will Do
To provide consultation and psychosocial counseling and support services to the Sharp Health Care customer base. To mobilize patients and families personal and environmental resources to receive maximum benefits of medical care and achieve the fullest personal and family functioning.
Required Qualifications
- Master's Degree in Social Work.
- Driver's License - CA Department of Motor Vehicles -REQUIRED
- AHA Basic Life Support for Healthcare Professional (AHA BLS Healthcare) - American Heart Association -REQUIRED

Essential Functions
Sets daily priorities and revises schedule based on patient / family needs.
Responds to crisis and issues in a timely manner.
Maintains contact with patient and families based on department guidelines, need and plan of care.
Provides counseling / education related to coping.
Provides counseling / education related to anticipatory grief.
Provides counseling / education / interventions related to urgent caregiver / safety issues.
Provides counseling / education related to end of life issues.
Provides patient centered care and promotes autonomy and patient advocacy.
Able to identify needs for additional community resources and able to refer appropriately.
Completes comprehensive psychosocial assessments within five days from start of service and assesses needs throughout time on service including, but not limited to :
Final arrangements; and
Assessment of bereavement needs upon death.
The clinician is able to document professionally and appropriately in regards to, but not limited to : Initial assessments, routine visits, clinical communication with team, and discharges.
Additionally, documentation is completed by end of shift and is individualized per patient.
Develops and revises plan of care based on client needs.
Performs services as ordered.
Documents according to plan of care.
Documents changes in plan of care.
Follows visit frequency and updates based on patient / family need and clinical judgement.
Identifies and clarifies professional role and performs within scope of practice.
Understands roles of other IDT members and able to appropriately refer to other disciplines.
Maintains ongoing communication with team members.
Demonstrates respectful and professional behavior with employees, patients, families and community partners.
Sensitive to cultural differences and practices culturally competent care.
