Job Description
Our client is seeking a skilled RN Case Manager to join their team on a temporary basis. The ideal candidate will possess excellent communication and interpersonal skills, with the ability to work effectively in a fast-paced environment.
Responsibilities and Expectations
- Provide comprehensive case management services to patients with complex health needs
- Collaborate with interdisciplinary teams to develop and implement patient care plans
- Assess patient risks and vulnerabilities, identifying potential areas of concern
- Develop and maintain effective relationships with patients, families, and caregivers
Skills and Qualifications
- Current Florida license
- At least 5 years of overall experience or 3 years of recent experience in Case Management
- Previous travel experience strongly preferred
