Shawnee Health – School Based Health Liaison

Shawnee Health is seeking a full-time School Based Health Liaison for our School Based Health Center Terrier Care and Carbondale District-95. Team member will be assigned to alternative Shawnee site when school is not in session. This position reports directly to the Nurse Manager responsible for School Based Health Centers.
Responsibilities
  • Prepares the exam rooms for the patient by setting up any needed equipment and maintains a clean, organized, efficient and confidential work area.
  • Updates immunizations and documents relevant information by following appropriate EHR work flow for the visit type and alerts provider to any concerns.
  • Performs point of care lab tests and venipunctures. Correctly processes orders for external lab pick-up, understands how to process, result, and document all labs including confidential.
  • Completes appropriate screenings as required by visit type and age.
  • Administers medications or immunizations as appropriate and documents in the patient’s medical record.
  • Serves as liaison between school staff, Shawnee Health, students and their families to help meet needs.
  • Assist students and families in utilization of resources including scheduling appointments and transporting students between school and Shawnee Health facility.
  • Have a working knowledge of available resources to ensure students and families have access to needed services.
  • Engage in the community and school district through outreach to identify and recruit target population high-risk students from schools and service areas
Requirements
  • High School Diploma or equivalent
  • Graduate of a Medical Assistant Program and proof of current certification
  • Completion of Community Health Worker Certification Course through recognized public health organization (Listed on CHW) within 180 days of employment with Shawnee Health
Preferred
  • 2 years of experience as a CMA
  • Bilingual
Starting salary – $21.10-$23.08

Shawnee Health offers two health insurance plans so that employees can choose a plan that is best suited for their needs. In addition to in-network plan offerings, Shawnee Health also offers a Tier 1 discount whenever employees utilize Shawnee Health services such as family medicine, behavioral health, nutrition, lab services, and same day appointments. The Tier 1 discount drops copays from $30 per visit to only $5 for most services.

Paid Time Off
  • Vacation – Full time employees earn 96 hours of vacation leave per year. After 3 years of employment, employees earn 120 hours. Every year thereafter, employees earn vacation leave up to a maximum 28 days per year in accordance with their length of service.
  • Sick – Full time employees earn 48 hours, or 2 hours per pay period, sick leave per year.
  • Personal – Full time employees receive 24 hours personal leave per calendar year. New hires receive a prorated portion of the allocated personal leave based on their hire date.
Employees working 60%-99%FTE earn a portion of the listed hours. Employees working less than 59% FTE qualify for PLFAW (Paid Leave for All Workers Act)
Call & Incentive Compensation
Care Coordinators, Case Manager Assistants, and Adult Protective Services Case Workers and Specialists are eligible for call compensation. Care Coordinators are eligible for incentive compensation.
Standard Benefits for Physicians, Dentists, and Midlevel Practitioners
  • Shawnee Health offers 280 hours for full time practitioners for each full fiscal year of employment for vacations, holidays, sick leave, continuing education or other absences from the practice. At the end of any employment year, a portion of unused Leave days may be carried forward to the next year or you may opt to take the cash value of the unused Leave days.
  • Productivity Incentives: Shawnee offers select physicians, dentists, and midlevel practitioners’ merit and productivity incentives based negotiated contractual terms related to net collections and/or total encounters.
  • Merit Bonus: Shawnee offers select physicians, dentists, and midlevel practitioners’ productivity incentives based on negotiated criteria.
  • Continuing Education (CE) Allocation: physicians, dentists, and midlevel practitioners receive an annual CE allocation.
Retirement Planning
We strive for our employees to live comfortably after employment. Shawnee Health contributes 3% of your annual salary. Shawnee Health Service maintains a 401(k) Retirement Plan for employees with 4 contribution types:
  • Salary deferral – Pre-Tax 401(k) deferrals or Roth 401(k) deferrals (after-tax).
  • Safe Harbor contribution.
  • Profit Sharing Non-Elective or Discretionary Contribution
  • Employee “rollover” contributions.
Educational Assistance
Our educational assistance program offers up to $10,000 per fiscal year towards completion of study, degree, certification, examination, license or similar educational pursuit which will provide a mutual benefit to the employee and SHS.
To be considered eligible, employees must have completed 1 or more years of continuous service with Shawnee Health and a minimum of 24 hours per week. Additionally, employees who receive education assistance are required to sign a Repayment Agreement that requires them to remain employed by Shawnee Health for a specific period of time. Contractual providers aren’t eligible to receive educational assistance, with the exception of loan repayment.
Student Loan Repayment Program
Our Student Loan Repayment Program offers up to $10,000 in loan repayment that can be paid in one lump sum or split into monthly payments. To be considered eligible, employees must have completed 1 or more years of continuous service with Shawnee Health and a minimum of 30 hours per week. Additionally, employees who participate in the Student Loan Repayment Program are required to sign a Repayment Agreement that requires employees receiving assistance to remain employed by Shawnee Health for a specific period of time.
