As a Primary Care NP, you will work collaboratively with physicians, nurses, and other healthcare professionals to deliver comprehensive primary care services, promote health education, and empower patients to achieve optimal wellness.
Key Responsibilities :
- Conduct comprehensive assessments, including health histories, physical examinations, and diagnostic tests, to evaluate patients' health status and develop individualized care plans.
- Diagnose and treat common acute and chronic conditions, such as hypertension, diabetes, respiratory infections, and minor injuries, in accordance with evidence-based practice guidelines and regulatory standards.
- Provide preventive care services, including immunizations, screenings, and health maintenance counseling, to promote wellness and disease prevention.
- Manage chronic illnesses through medication management, lifestyle modifications, and patient education, ensuring continuity of care and adherence to treatment plans.
