Medical Director Opportunity
About the Role:
We are seeking a skilled Physician Leader (MD/DO) to serve as a Medical Director in our post-acute care setting. This full-time position involves patient care rounding and medical director responsibilities.
The ideal candidate will possess a high level of autonomy and enjoy addressing complex problems within an interdisciplinary medical team. They will work closely with two rehabilitation facilities, rounding approximately 3.5 - 5 days per week with flexibility to manage their daily schedule.
Key Responsibilities:
- Patient care rounds in rehabilitation facilities
- Maintenance of chronic disease management, diagnosis of illnesses, and implementation of care plans
- Collaboration with clinicians, facility leaders, and others regarding care and services
- Medical director duties for assigned facilities
- Clinical case reviews, sharing tools, information, and guidelines related to quality of care
- Collaboration with leadership to review care coordination initiatives, develop collaborative intervention plans, and share ideas on network management issues
Required Qualifications:
- Minimum 2 years of primary care experience, with post-acute experience being beneficial but not required
- Leadership background is a plus, but training will be provided
- Active and unrestricted New York medical license or application in process
- Active and unrestricted DEA license or application in process
