Job Title: Case Manager I
Reports to: Executive Director
Job Status: Part-Time regular employee / Up to 20 hours per week / Benefit eligible
Pay: $21.00 - $24.00/hour

Position Summary

Bridgeway is a transitional living program for single women with children who are homeless or are living in unsafe or unstable environments, dedicated to restoring hope through caring and loving relationships. We are looking for a passionate professional who is willing to take a trauma focused and strength based approach to supporting residents, aftercare clients and outreach contacts of the Bridgeway program. They will build a strong team environment based on mutual respect, transparency, open communication, and support. The Case Manager I must utilize strong interpersonal, organizational, advocacy, problem solving, and decision-making skills daily, as well as effectively lead, coach, and mentor employees, residents and volunteers. Because of the nature of the program, some flexibility is necessary.

Essential Duties and ResponsibilitiesCase Management (75%)

  • Meet with Bridgeway residents weekly to set goals and monitor program progress
  • Assess potential Bridgeway candidates and conduct intake meetings
  • Co-facilitate programming with the Case Manager II and House Manager and contribute to programming and a variety of other life skills topics including parenting, communication, self-esteem, healthy- boundaries
  • Active participant in the Bridgeway Team (Case Manager II and House Manager) to ensure strong communication and program consistency
  • Follow-up with clients and agencies, input accurate and complete data for all contacts with clients into agency database
  • Provide all required documentation in a timely manner, which may include client follow-up, client intakes, client case notes, reports and evaluations
  • Adhere to agency policies, procedures and professional code of ethics

Community Engagement & Leadership (25%)

  • Participate in regular staff meetings, staff training programs, and supervisory sessions
  • Contributes to maintaining the SCIO property
  • Establish ongoing collaborations with community organizations and groups
  • Will represent SCIO and Bridgeway in the community, including presentations and community meetings
  • Work collaboratively with the SCIO leadership team to complete required reports
  • Other duties as assigned

Desired Skills, Education & Experience

  • Degree or experience in Human Services, Psychology, Social Work or related field, or related experience
  • Knowledge of community resources, public benefits, and counseling/social work practices with high risk populations. Experience working with persons in crisis.
  • Experience and proficiency with Microsoft Office, Google documents and Gmail
  • Excellent written and verbal communication skills with the ability to lead difficult conversations
  • Ability to work independently with a strong sense of focus, task-oriented, nonjudgmental, open personal qualities, clear sense of boundaries.
  • A strong sense of and respect for confidentiality and handling sensitive information appropriately.
  • Ability to work in a variety of settings with culturally-diverse families and communities with the ability to be culturally sensitive and appropriate.
  • Trauma Informed Care knowledge and strengths-based approach preferred.

Other Job Requirements

  • Valid driver’s license
  • Ability to pass a background check & pre-employment drug screen required
  • Ability to use up to 50 pounds of force occasionally, and/or up to 20 pounds of force frequently and/or up to 10 pounds of force continuously to move objects and/or people
  • Ability to bend, stoop, twist, turn, reach, lift, carry, pull, climb, and kneel, with frequent walking and standing
  • Ability to recognize differences in sound, such as voices/noises that are loud and playful instead of angry and combative, and the ability to differentiate tone and volumes in conversations
