What are the responsibilities and job description for the Charge Nurse - CHANT (Health Services) position at Shelby County Government?
Position Summary
This position is funded by a grant for a duration of time as defined by the grant. Responsible for the overall supervision of a care coordination program aimed at improving the health of vulnerable populations, including pregnant women and children under 21 years of age who are eligible for or enrolled in TennCare. This program specifically addresses the needs of those with special health care requirements.
Pay Grade: 53
Salary Commensurate with Experience and Education
Minimum Qualifications
1. Three (3) years of nursing experience; of which one (1) must be in a lead or supervisory capacity; AND
2. Bachelor's degree from an accredited Nursing program; AND
3. Must possess or obtain an active RN license in the State of Tennessee by date of employment.
4. PROOF OF EDUCATION, TRAINING, AND/OR EXPERIENCE IS REQUIRED.
Duties and Responsibilities
1. Collaborates with program’s leadership to oversee the development and implementation of the program in accordance with programmatic guidelines and provides leadership and assists in supervising care coordinators with the daily operation of a care coordination program.
2. Ensures compliance with state-mandated policies, guidelines, and timelines. Responsible for approving or denying services and providing staff with appropriate programmatic information.
3. Reviews and approves certifications and pending certifications for the program in accordance with the state CHANT policy guidelines.
4. Assists in reviewing and approving treatment plans and service and pharmacy reports.
5. Assures accuracy of records and documents completed by care coordinators for all family service plans and program referrals (e.g. proof of income, residency, insurance, and up to date physicals).
6. Links pertinent clinical information to the care coordinator and assists in the development of plans of care to address families enrolled in the program’s pathways.
7. Assists Supervisor Public Health Nurse in conducting quarterly audits, record reviews, and monitoring care coordination activity reports.
8. Educates and empowers the public through attendance at targeted health fairs, community agencies, hospitals, clinics, and healthcare providers informing of program’s services.
9. Monitors health, wellness, and access for infants with abnormal newborn screenings by following up on all abnormal newborn screening referrals and forwarding report as requested per State mandate.
10. Obtains medical records from CIOX and assists the PHN supervisor in reviewing and approving claims.
11. Provides support, reflective supervision, coaching, training, and technical assistance for staff. Completes quarterly program evaluations for navigation and engagement activities.
12. Participates in monthly CHANT staff meetings and huddles, attends CHANT field staff meetings as required by the Tennessee Department of Health Central Office to discuss programmatic issues; and assists in development of policies to better serve program enrollees.
13. Completes special formulas and food orders for children with special needs and ensures monthly monitoring of orders.
14. Performs other related duties as required or directed.
KSAs
1. Excellent knowledge of the principles of basic nursing.
2. Advanced knowledge of health and social service agencies and of community resources; excellent communication skills with clients and professionals; working knowledge of basic medical terminology, ability to organize, implement, and maintain a tracking system.
3. Knowledge and understanding of goals, purposes, policies, and practices of the CHANT program.
4. Ability to educate parents, other professionals, and public of chronic illnesses and medical conditions affecting children’s ability to function independently.
5. Ability to provide ad hoc reports per State Directors request.
6. Ability to keep staff aware of policy changes affecting their job duties and responsibilities.
7. Ability to communicate effectively in written and verbal form with diverse audiences.
8. Able to use good judgment in evaluating situations and making decisions in absence of CHANT supervisor.
9. Proficient in the use of MS Office (Excel, Word, PowerPoint) or similar software.
Physical Requirements
1. Must be able to perform physical activities.
2. Work will require lifting and transporting materials and supplies with the ability to exert up to 25 lbs.
Special Requirements
1. Must possess a valid Tennessee driver license by date of employment.
2. Must have daily access to an operable automobile and meet county requirements for automobile insurance upon the date of employment.
3. Must have or be able to obtain an active unencumbered RN license in the State of Tennessee by date of employment.
4. Must be willing to carry a company issued cellular phone and iPad.
5. Work will require attendance at field staff meetings as required travel to locations outside of the office: home visitation community agencies, schools, and doctor offices.
6. Must maintain all required certifications and licenses throughout the duration of employment.
