Health Investigator - INFECTIOIUS DISEASE (Health Services)

This position is eligible for a HIRING BONUS outlined below:

1st payment of $1,500 paid after completion of six (6) months of service

2nd payment of $1,500 paid after completion of twelve (12) months of service

3rd payment of $2,000 is paid after completion of eighteen (18) months of service.

Minimum Qualifications

1. Four (4) years of experience in customer service, health services, education, community outreach, social service industry, or conducting intake interviews; AND

2. High school diploma or possess a GED; OR

3. Two (2) years of experience in customer service, health services, education, community outreach, social service industry, or conducting intake interviews and an associate degree from an accredited college or university; OR

4. Bachelor’s degree or higher from an accredited college or university.

5. PROOF OF EDUCATION, TRAINING, AND/OR EXPERIENCE IS REQUIRED.

Duties and Responsibilities

1. Obtains and enters positive laboratory reports from laboratory, clinical providers, and other facilities.

2. Communicates with providers to obtain missing information.

3. Contacts the patient for follow-up and treatment.

4. Performs outreach STD screening/testing activities.

5. Participates in provider and patient education about STDs and their treatment.

6. Conducts provider visits to collect reports, to assure the quality of reporting, and to provide new information and assistance.

7. Completes required monthly reports.

8. Conducts field visits interviews and partner notifications as they relate to outbreaks and illnesses.

9. Performs other related duties as required or directed.

KSA's

1. Knowledge of medicine and communicable and infectious disease treatment and control.

2. Awareness of the principles of public health and of the objectives of public health programs.

3. Strong verbal and written communication skills.

4. Ability to work courteously with public citizens and co-workers.

5. Ability to exercise good judgment in evaluating situations and making decisions.

6. Ability to express ideas clearly and concisely.

7. Proficiency in MS Office (Outlook, Word, Excel) and familiarity with other County’s software applications.
