What are the responsibilities and job description for the Intake Assessment Specialist - CHILDHOOD LEAD (Health Services) position at Shelby County Government?
Position Summary
This position is funded by a grant for a duration of time as defined by the grantor.
Position is responsible for performing intake assessments and making recommendations for individuals referred to the Healthy Homes Assessment Program. Reviews Housing Urban Development (HUD) policies, and procedures to determine client income eligibility, and employment verification for home remediation.
Pay Grade: 48
Salary Commensurate with Experience and Education
Minimum Qualifications
1. Three (3) years of experience in social work, case management, social services, healthcare, or a closely related field, AND
2. Bachelor’s degree from an accredited college or university in social work, sociology, psychology, or a closely related field, OR
3. An equivalent combination of related education and/or experience.
4. PROOF OF EDUCATION, TRAINING, AND/OR EXPERIENCE IS REQUIRED.
Duties and Responsibilities
1. Receives referrals from Single Point of Entry (SPOE) for potential non-Medicaid applicants or MDS Section Q referrals while conducting initial, follow-up, functional, and social assessments, utilizing a statewide tool to determine applicant’s eligibility and enrollment criteria for CHOICES.
2. Counsels potential enrollees and family members about the CHOICES program, which includes explanation of program and qualifications for CHOICES, limits on enrollment, and Federal Estate Recovery Program.
3. Responds to questions regarding information provided by intake and assessment staff.
4. Begins the Pre-Admission Evaluation (PAE) application process during intake site visits using the online TPAES system.
5. Facilitates the Medicaid application process during the intake site visit by collecting all appropriate DHS-required documents and identification.
6. Maintains individual caseloads and reports, gives weekly updates to management on the application status until participant’s enrollment status is complete.
7. Documents actions to applicant’s electronic files and closes files for incomplete applications or for non-receipt of required documents.
8. Completes Pre-Admission Screening Reviews in homes and in long-term care facilities as needed.
9. Contacts enrollee within 24 hours of receipt of assigned referral to schedule site visit and conducts visit.
10. Completes consumer satisfaction surveys.
11. Follows up with the process until the consumer’s history and physical (H&P) are received.
12. Performs chart audits for accuracy.
13. Performs other related duties as required or directed.

5. Proficient knowledge in Microsoft Office (Word and Excel) and other County’s software and computer applications.
Special Requirements
1. Must possess a valid Tennessee driver's license or secure one as of employment date.
2. Must have daily access to an operable automobile and meet county requirements for automobile insurance upon the date of employment.
3. Must be available on weekends and/or after regular work hours.
