Shelbyville Manor is seeking a qualified Social Service Director for our beautiful retirement community. Social Service Director's Responsibilities:
1. Serve as a liaison between facility, residents, responsible parties, and
outside agencies
2. Refer resident to outside agencies as appropriate.
3. Provide information to resident/responsible parties as to Medicare/Medicaid,
and other financial assistance programs available to the resident in conjunction
with the Business Office Manager.
4. Keep abreast of current state and federal regulations, professional standards,
and facility policy and procedures.
5. Attend meetings as required.
6. Facilitate grievance process in conjunction with the Grievance Officer
7. Provide social support for residents as needed.
8. Complete observations and additional documentation as required.
9. Complete MDS sections as assigned, ensure all areas are addressed in the
care plan as necessary.
10. Provide assistance to Activity Director in addressing mood and behavioral issues.
11. Participate as a member of the Interdisciplinary Team.
12. Provide back-up to the Admissions Director with Admission and Discharge planning process
13. Perform other duties as assigned by the supervisor
Social Cervice Director's Qualifications:
Education: Bachelor’s degree in Human Services Field
Experience: One (1) year of supervised experience in Social
Services field
