POSITION TITLE:   Sr. Care Coordinator

STATUS:                  Non-exempt/Full Time

TEAM:                     Shelter Services

REPORTS TO:         Shelter Manager or Director

SUPERVISES:         None

SUMMARY: This position is responsible for working with and consulting with health challenged participants and assisting in determining wellness needs.  Provide medication monitoring and assists with first aid services as needed. This position may also assist Case Managers in coordinating patient-care health services.

ESSENTIAL FUNCTIONS & RESPONSIBILITIES:

RESPONISBILITIES:

  • Conduct interviews with participants to access and gather information for factual basis on which to formulate a plan in conjunction with Case Managers.
  • Ongoing consultation with participants and their respective Case Managers to discuss health concerns.
  • Provide supervision and oversite to the medication monitoring process, including setting up medication calendars, tracking participant usage, refill reminders and pharmacy databases.
  • Work closely with participants to educate them on their current medical condition(s), diagnosis and medication(s), including best practices, potential side effects etc.
  • Independently counsel participants on mental health, drug and substance abuse.
  • Work with the Sr. Case Manager and assist in developing a plan of care to address participants personal health care needs.
  • Consult and collaborate with other healthcare providers and specialists to set patient appointments or follow ups, as needed.
  • Provide wellness checks on participants regularly and evaluate and document progress. Work with healthcare providers and Case Managers as needed.
  • Assist the participants’ care team with developing and assessing health interventions.
  • Attend ongoing training and courses to keep abreast of new developments in health care & case management.
  • Comply with organizational guidelines and health care laws and regulations.
  • Perform routine comprehensive, independent reassessment of participant status and progress toward achievement of plan goals. Make adjustments in writing as appropriate.
  • Utilize existing community resources in providing care/services to meet goals.
  • Participate in internal and external case conference/case coordination meetings.
  • Connect and maintain liaison to internal & external service resources
  • Keep all relevant service data up-to-date in HMIS system and maintain case files in accordance with applicable guidelines, completing data entry immediately or no later than within 24 hours of service interaction.
  • Comply with data integrity, HIPAA, 42 CFR 2, and security policies.
  • Meet data quality standards by reviewing discrepancies in data received, verifying accuracy, requesting clarification and advising supervisor of issues related to data.
  • Other duties as assigned

COMPETENCIES:

  •  Knowledge of medical systems including medical office processes and practices.
  • Ability to screen and check for eligibility of MediCal and Partnership Health.
  • Advanced knowledge of medical conditions and medications used in treatment, including potential side effects.
  • Demonstrate knowledge and understanding of participant health and social services presenting risks and apply appropriate risk stratification.
  • Familiarity with drugs of abuse, drug-seeking behaviors and doctor shopping.
  • Knowledge and skills in handling substance abuse and mental health issues.
  • General knowledge of common health concerns such as Diabetes, food allergies, heart disease, common GI afflictions etc.
  • Knowledge of holistic medicinal practices, such as Native American and Eastern philosophies.
  • Knowledge of public benefits and financial resources available in the community.
  • Crisis intervention and conflict resolution skills including use of motivational interviewing, harm reduction approach, and trauma-informed care.
  • Thoroughness and accuracy with data collection, entry and quality control in a web based database. 

MINIMUM QUALIFICATIONS & REQUIREMENTS:

  • Associates degree in social services or related area, certification from a two year program in related area, and/or a combination of education and experience that demonstrates a capacity to help people coming from crisis situations and traumatizing circumstances to develop self-sustainability.
  • Experience working with low income and/or homeless populations.
  • Proficient computer skills, including Microsoft Office Suite (Outlook, Word and Excel)
  • Proficient typing skills and working knowledge of medial chart management and storage.
  • Clear/firm-yet-flexible boundaries, consistent energy level and positive demeanor.
  • Ability and willingness to work flexible hours to accommodate participants available during the evening or on weekends.
  • Experience working in diverse settings with people across all socio-economic spectrums and a wide variety of personalities and roles - staff, residents, local agencies, contractors, lenders, etc.
  • Professionalism: high level of integrity and strong ethical values show capacity to maintain highest standards of confidentiality with all records, including organizational and individual information.
  • Strong oral/written communication and listening skills.
  • Self-motivated and accountable for work time and other agency resources.
  • Quality control: demonstrates accuracy and thoroughness, monitors own work to ensure quality and applies feedback to improve performance.
  • Well organized: able to effectively manage multiple assignments to meet project deadlines.
  • Access to reliable personal transportation required, including a DMV record that permits driver to be insured under SHELTER, Inc.’s automobile coverage.
  • Must successfully pass a criminal background check.

PREFERRED QUALIFICATIONS:

  •  Bachelor’s Degree with emphasis in social services, psychology or related fields
  • Experience working with people in low-income subsidized housing arrangements and/or supportive housing programs.
  • Bi-Lingual English Spanish

Specific vision abilities required by this job include close vision.

This position is typically done indoors in an office setting but the employee may need to travel to different locations.

The employee must frequently lift and/or move up to 10 pounds and occasionally lift and/or move up to 25 pounds.
