HCRP Case Manager

Under the supervision of the HCRP Supervisor, this position develops and implements the Individualized Service Plan (ISP) for clients. Case Manager will provide information and referrals for: housing, medical, legal, employment, educational, mental health, substance abuse, financial support and other needs.

Essential Duties and Responsibilities

* Develops Individualized Case Management Plan with all guests

* Works with community landlords

* Educates client’s on Landlord/Tenant rights

* Case Manager will provide information and referrals for: housing, medical, legal, employment, educational, mental health, substance abuse, financial support and other needs.

* Maintains progress notes that indicate actions and activities related to case plan

* Maintain current/update client files

* Maintains contact with agencies in the community that can provide services and programs for guests

* Ability to learn and implement the program standards and regulations

Develop partnerships and relations with utility companies

* Assist in scheduling routine inspections

* Complete payment order request

* Presents and attends case conferences and meetings

* Works with guests to maintain rules and procedures of program

* Conduct home visits

* Complete guest intakes and prepares other documents as needed

* Turn all reports/client’s database in timely manner and correct

* Performs other duties as assigned

Qualifications: Requires a minimum of two years of higher education in social work or relevant in field experience. Good written and oral skills. Must be energetic and have positive attitude and ability to interact well with guests, staff, and colleagues at other agencies.

Job Type: Full-time

Pay: From $17.00 per hour

Expected hours: 40 per week

Schedule:

• 8 hour shift
• Day shift
