Job Overview
We are seeking a Support Coordinator to join our team. The ideal candidate will assess the needs of a client and effectively, plan, manage, coordinate, and follow up on services which most appropriately meet the identified needs as mutually defined by the client, caregiver, family, professionals, and other supports.
Responsibilities
- Help individuals understand and use their plan to achieve their goals
- Help individuals connect with community resources, government services, and other providers
- Help clients make informed decisions by acting as their advocate regarding their service option
- Help individuals build skills to coordinate their support and live more independently.
- Coordinate meetings and events
- Record case information, complete accurately all necessary forms and produce statistical reports
- Promote to professional standards as outlined by protocols, rules and regulations
Skills
- Proven working experience in case management or a related filed preferred
- Strong organizational skills and time management skills
- Effective communication skills
- Previous experience with developmental disability preferred
- Ability to multitask and prioritize tasks effectively
- Compassionate with teamwork skills
